 We want to make your first visit at Anne Arundel Dermatology as pleasant and comfortable as possible. To better serve you please bring your insurance card with you. If your insurance requires you to have a referral, please bring that with you at the time of your visit. You will be asked to fill out a Patient Enrollment Form and a Medical History Form. To save time please print and complete the Patient Enrollment Form.
For No Show/Cancellation:
We assess a charge for missed appointments and late notice of cancellation. We acknowledge that our patients may have to wait longer than they would like for an appointment with our physicians. When patients don’t show for their appointments, we lose valuable time. Time that another patient would gladly have used and time our doctors wait for a no show. It is not our preference to charge for no shows and late cancellations; we regret we must take such action as a deterrent. Referrals:
If your insurance company requires that you have a referral in order to see a specialist; please obtain one from your Primary Care Physician (PCP) well in advance of your appointment. Some PCPs offices require up to 72 hours notice to process a referral. The referral is your responsibility. Our physicians cannot see you without a referral, if your insurance company requires one. Co-Pays are due at the time of your appointment. You may pay by cash, check or credit card. We accept Mastercard, Visa.
